Sister's Keeper - Motivations

While we're all pretty sure that everyone in this book is really trying to do the best thing they can, why are they all doing what they're doing?

Sara, Anna and Kate's mother, is trying to save Kate's life.
Brian is trying to keep his family together.
Anna is trying to allow the medical decisions to be made by the person they affect most.
Jesse is trying to get his parents' attention, and to support his sibs.
Campbell is trying to generate some relatively free publicity.
Julia is trying to protect Anna.

Who am I missing? And, do you agree with my evaluation of these characters' motivations?

Under the circumstances, could any of the characters behaved in a different manner? How would the book be different if the characters made different decisions?
